How it works

1. Referral

You may be referred by a family member, friend, doctor, or therapist, or contact Noor Adult Day Health Care Center directly to learn about our services and schedule an intake interview.

2. Intake interview

Our staff will meet with you to assess your eligibility and your needs for assistance.

3. Eligibility

You may qualify if you have conditions or illnesses that threaten your quality of life or ability to remain independent, and you require health care and/or therapy services without the need for 24-hour care.

4. Home visit

A social worker will arrange a visit to your home. Your medical history will be obtained from your personal physician or through an examination by our staff physician.

5. Assessment

Our clinical staff complete in-depth assessments at the center to identify the services you need and how often you should attend.

6. Care plan

Once you qualify, you will receive your individualized plan of care. Your attendance schedule will be set, and transportation arranged as needed.
